The flu vaccine is free to NIU employees on any of the State of Illinois Health Care Plans, as well as retired employees and survivors with State of Illinois Coverage who are not yet eligible for Medicare. Spouses, dependents and students will be a cost of $38. Students are encouraged to go to the University Health Services for a reduced cost for their vaccine. Any questions can be directed to the Employee Assistance Program and Training Center at 815-753-9191.

The NIU EAP Health Events webpage has a printable version out the Vaccine Administration Record that must be completed and brought to the clinic. Blank forms will be available at the clinic.
