NIU to host Flu Shot Clinic

DeKALB —The DeKalb County Health Department, the NIU Employee Assistance Program, and the NIU Nursing Assistance Clinic are hosting a flu shot clinic Thursday from 10 a.m to 4 p.m. at the Holmes Student Center, Duke Ellington Ballroom. The clinic is free and open to the public.

Participants are encouraged to schedule an appointment online, pre-fill out the client form, and bring a copy to the clinic.

Attendees should enter the east side of the ballroom and exit through the main door for safety and social distancing measures. Face masks are required.

Students need to bring a completed and signed client form, an insurance card and a student ID. 

NIU employees should complete the A, B, and E portions on the form, bring an insurance card, and a photo ID. If an employee or student is opting out of insurance, fill out A, C, E on the form. 

Retirees need to fill out A, C, D, and E on the client form and bring either a Medicare card, Medicare advantage card, or supplemental insurance card and photo ID.
